New Client Info & Form
To begin the process of becoming a client with us, we ask that prospective clients fill out our new client web form. If you do not hear back from us, this means that you are on our waitlist and will be contacted within a few weeks typically to schedule your initial client appointment, which is billed by insurance and is subject to client’s cost per their insurance coverage. Once fit is determined, you will be scheduled for regular appointments with the practice.
You may hear back from us after filling out the new client form, and the reason for this would be that we could not place you on our waitlist due to issues with insurance coverage, or if your needs are outside of our clinical scope and/or level of care.
